Prestige Health Choice Remote Biometric Monitoring Program Now Available in Orlando and Surrounding Areas
Telehealth platform gives members living with congestive heart failure or uncontrolled diabetes the ability to manage their condition from home
Prestige Health Choice, a Medicaid managed care plan serving 55 of Florida’s 67 counties and a joint venture between AmeriHealth Caritas and Florida Blue, has expanded the footprint of its remote biometric monitoring program into Brevard, Manatee, Orange, Osceola, Sarasota, Sumter and Wakulla counties, giving more health plan members living with congestive heart failure or uncontrolled diabetes mellitus a new way to monitor their health status.
The program, a collaboration with Vheda Health, launched in Pinellas, Hillsborough, Pasco, Polk and Highlands counties in October 2017. Vheda Health sends participants a cellphone, weight scale, blood pressure monitoring cuff, and pulse oximeter to measure oxygen values. Pilot participants diagnosed with diabetes mellitus also receive a blood glucose meter for daily glucose checks.
Should the health data collected from these devices be higher or lower than a pre-determined threshold, the member can work with his or her physician, Prestige Health Choice care manager and a Vheda Health coach to address any issues, with the goal of avoiding emergency department visits or hospital admissions.
“We are thrilled to see how readily this technology has been adopted by our members, particularly among those living with a complex chronic condition who require stringent medication compliance,” said Prestige Health Choice Medical Director Sandra Schwemmer, D.O. “Analysis of readmission data indicates that congestive heart failure and diabetes are responsible for a large number of 30-day hospital readmissions. Through this program, we hope to reduce the number of readmissions.”
