Health Risk Assessment

A Health Risk Assessment (HRA) helps us understand your health care needs. An HRA form was mailed to you in your new member welcome packet. Completing this form should take just a few minutes.

If you can’t find your form, you can:

  • Download the HRA (PDF), complete it, and fax it to 1-855-236-9281.
  • Call our Rapid Response and Outreach Team at 1-855-371-8072. We will take your information over the phone and complete your HRA for you.
  • Call Member Services at 1-855-355-9800 (TTY 1-855-358-5856) to get a new HRA mailed.

We will not share your HRA with anyone other than your Care Manager.
