Healthy Behaviors Programs

Good health is important to all of us. We want you to be in the best health and will encourage you to reach your goals. There are several different Healthy Behaviors programs. Let’s get started on choosing the right program(s) for you.

Get rewarded for reaching your health goals

You can earn rewards by completing a program below. You may complete more than 1 program if you qualify.

  • Weight loss.
  • Smoking cessation.
  • Alcohol and substance use recovery.

If you would like to join, bring a Commitment Form (PDF) to your next primary care provider (PCP) appointment. Your PCP can indicate which program(s) you would like to complete and sign your Commitment Form. PCPs can fax the Commitment Form to the Prestige Health Choice (Prestige) Rapid Response and Outreach Team at 1-855-236-9281, or you may mail the signed Commitment Form in the prepaid envelope provided.

More healthy behaviors

You can also earn rewards for additional healthy behaviors:

  • Dental — Members must complete at least one dental visit to qualify for the reward. Member must be 2 to 20 years of age as of December 31, 2017.
  • Maternity — Member must complete at least 10 prenatal visits, and a postpartum visit within 21 to 42 days of giving birth. Member must have delivered a child on or between November 6, 2016, and November 5, 2017.
  • Postpartum — Member must have a postpartum visit within 21 and 42 days of giving birth. Member must have delivered a child on or between November 6, 2016, and November 5, 2017. Members who complete the Maternity Reward Program will not be eligible for the postpartum reward program as well.
  • Well child (for members who have turned 15 months of age during 2017) — Member must complete at least six well-child visits with a doctor during the first 15 months of life. Member must turn 15 months old on or between January 1, 2017, and December 31, 2017.
  • Well child (for members who are 3 to 6 years of age during 2017) — Member must complete one or more well-child visits with a doctor. Member must be 3 to 6 years of age on or between January 1, 2017, and December 31, 2017.
  • Adolescent well care (for members who are 12 to 21 years of age during 2017) — Member must complete at least one well-care visit with a PCP or an OB/GYN practitioner. Member must be 12 to 21 years of age on or between January 1, 2017, and December 31, 2017.
  • Lead screening — Member must complete one or more capillary or venous lead blood test for lead poisoning. Member must be at least two years of age on or between January 1, 2017, and December 31, 2017.
  • Breast cancer screening — Member must complete one mammogram screening for breast cancer. Member must be 50 to 74 years of age on or between January 1, 2017, and December 31, 2017.
  • Cervical cancer screening — Member must complete one cervical cytology screening for cervical cancer. Member must be 21 to 64 years of age on or between January 1, 2017, and December 31, 2017.
  • Diabetes testing — Members with diabetes must complete the following diabetes testing series with their PCPs:
    • Hemoglobin (HbA1C) blood test.
    • Microalbumin/urine screen/monitor (kidney disease screening).

Member must be 18 to 75 years of age on or between January 1, 2017, and December 31, 2017.

  • Diabetes eye exam — Member with diabetes must complete the following diabetes testing with their PCPs:
    • Retinal or dilated annual eye exam.

Member must be 18 to 75 years of age on or between January 1, 2017, and December 31, 2017.

  • Behavioral health follow-up — Member must have a follow-up visit with their provider within seven days of discharge from an outpatient visit, an intensive outpatient encounter or partial hospitalization with a mental health practitioner. Member must be at least six years of age on or between January 1, 2017, and December 31, 2017.

Your provider can indicate which Healthy Behaviors program(s) you completed on the Completion Form. The Completion Form can be faxed to the Prestige Rapid Response and Outreach Team at 1-855-236-9281, or you may mail the signed Completion Form in the prepaid envelope provided.
